How to fill in a Word fax cover sheet

Download the .docx for the design you want.

Open it in Word and click into each field to type your details.

Save or print. Print it, or go to File > Save As and choose PDF to fax it online.
Add your logo
Go to Insert > Pictures and choose your logo. A PNG with a transparent background looks cleanest in the header. Save the file with your logo and details filled in, and you’ve got your own letterhead cover sheet.
